CADISS
(2019)Objective
To compare the effectiveness of antiplatelet therapy versus anticoagulation therapy in preventing recurrent stroke in cervical artery dissection.
Study Summary
• The trial found no significant difference between antiplatelets and anticoagulants in reducing stroke risk in patients with cervical artery dissection..
Intervention
Aspirin vs. Heparin followed by Warfarin.
Inclusion Criteria
Patients with cervical artery dissection confirmed via imaging, randomized within days of symptom onset.
Study Design
Arms: Antiplatelets vs. Anticoagulation
Patients per Arm: Antiplatelets: 125, Anticoagulation: 125
Outcome
• Stroke incidence: 2.5% in both groups.
• Recanalization rates at 3 months: 35% complete recanalization, 35% with persistent irregularity, 16% developed dissecting aneurysms, and 25% remained occluded.
• Anticoagulation was discontinued in many patients after 3 months (decreasing from 50% to 18.4%)..
