Clopidogrel plus aspirin versus aspirin alone to reduce microembolic signals in patients with recent stroke or TIA and symptomatic intracranial stenosis.
Study Summary
• In patients with symptomatic intracranial stenosis and microembolic signals, dual antiplatelet therapy with clopidogrel plus aspirin significantly reduced microembolic signals compared to aspirin alone over 7 days.
Intervention
Clopidogrel (300 mg loading, then 75 mg/day) + aspirin (75–160 mg/day) vs. aspirin alone (75–160 mg/day) for 7 days. TCD monitoring was performed on days 2 and 7.

• Primary: Microembolic signals on day 2 — 31.1% (dual) vs. 54.0% (aspirin); RR reduction 42.4% (95% CI 4.6–65.2); p=0.025
• No severe systemic or intracranial hemorrhages
• Minor hemorrhages in 2 patients in dual therapy group
• No severe systemic or intracranial hemorrhages
• Minor hemorrhages in 2 patients in dual therapy group
